HUD money available in Homeowner Lead Hazard Control Program

The Polk County Health Department, in partnership with the Dallas County Public Health Department, recently received a HUD grant for lead-based paint hazard control and healthy homes projects.

Over the next three years, the Polk County Health Department will provide services to 10 units — $15,000 for lead and $5,000 for healthy homes repairs per unit — for Dallas County residents whose homes were constructed prior to 1978, who have a child under the age of 6 living or visiting on at least two different days per week and who meet the HUD income requirements.

Both homeowners and landlords are eligible to apply. Housing selected for assistance under the program must be privately owned and constructed prior to 1978. There must be a child under the age of 6 living or visiting on at least two different days within a week, provided that each day’s visit lasts at least three hours, with combined annual visits amounting to at least 60 hours.

Income and other requirements are as follows:

The following documents will need to be submitted with an application to the Homeowner Lead Hazard Control Program:

  • Photo ID and Social Security cards for anyone in the household who is over age of 18.
  • three months most recent paystubs for anyone working in the household. (If self-employed, tax returns and W2s from the last two years are needed.)
  • If anyone in the household is receiving child support, please submit documentation such as a divorce decree, Child Support Recovery Unit statement or other proof of support.
  • If anyone in the household is receiving unemployment, a statement from the Iowa Workforce Development office is required.
  • If anyone in the household is receiving any other type of income (pension, FIP, rental income, etc.), appropriate documentation for that income source is required as well.
  • Three months of most recent bank statements for any checking or saving accounts
  • Utility bills showing paid up to date (water and electricity)
  • Copy of mortgage statement
  • Copy of homeowner’s insurance
  • Children under the age of 6 will need to have had a blood test within the last year. Blood tests are available from your family physician or at New Opportunities, 1-800-642-6330.

Please note: Files that are incomplete will be placed in an “Inactive Status” until all required documents are received.
