A Minburn woman died and a Minburn child was injured in a head-on collision south of Perry Tuesday night.

A Minburn woman was killed and a 7-year-old child was injured Tuesday night in a head-on collision south of Perry.

Danyel Denise Hardisty, 40, of Minburn was pronounced deceased at the scene, according to an Iowa State Patrol (ISP) crash report. Javen Sorenson, 7, of Minburn was injured in the crash.

According to the crash report, Hardisty and Sorenson were traveling northbound on J Avenue (County Road P58) about 9:45 p.m. in a 2001 Toyota Echo when a 2009 Volkswagen Jetta, driven by Stephen Joseph Wink, 53, of Council Bluffs and traveling southbound on J Avenue, “crossed over into the northbound lane and crashed head-on into” Hardisty’s vehicle.

“Impairment is suspected at the time of the crash,” according to the crash report. “Next of kin has been notified.”

Sorenson was transported to Blank Children’s Hospital in Des Moines by Dallas County EMS. Wink was transported with non-life-threatening injuries to the Methodist Medical Center in Des Moines by Dallas County EMS.

The Perry Police Department, Dallas County Sheriff’s office, Perry First Responders and Perry Volunteer Fire Department also responded.
