Yolanda S. Collins, 35, of Waukee was arrested Friday on charges of neglect or abandonment, assault causing serious bodily injury and dependent adult abuse.

A Waukee woman was arrested on a Dallas County warrant Friday in connection with a June incident in which she allegedly harmed an institutionalized West Des Moines man in her care.

Yolanda S. Collins, 35, of 2308 S.E. Sydney Lane, Waukee, was charged with neglect or abandonment, assault causing serious bodily injury and dependent adult abuse.

According to court records, Collins was working June 1 as a medication aid in a West Des Moines assisted living facility when she had interactions with a “dependent adult” with “low cognitive function.”

According to court records, the actions by Collins allegedly caused the victim “to fall to his side and strike his abdomen on a shower chair,” and he “was left lying on the floor in apparent pain” for about “two minutes.”

Collins allegedly “reported to her employer that the victim was uninjured,” but four days later “the victim was found to have several broken ribs” as a consequence of the incident.

“Surveillance video from the room” showed the course of the events, according to court records.

A warrant for Collins arrest was ordered June 26 in Dallas County District Court and served July 11. Collins was held in the Dallas County Jail on a $10,000 cash or surety bond. She is scheduled for a preliminary hearing July 18 in Dallas County District Court.
