Health department seeks input on community plan with survey

The Dallas County Health Department is seeking input from its community partners as it develops the 2023-2025 Community Health Improvement Plan.

County residents are invited to spend about 15 minutes on a survey that will provide the health department with thoughts on the top health priorities for Dallas County.

In 2021 health system and public health leaders from Polk, Warren and Dallas counties hired a research firm to conduct a health assessment of the region. Eleven areas of opportunity for the region were identified, and the survey now seeks input about what people think are the top health priorities in Dallas County and what action steps to take to accomplish them.

The feedback from the survey will be used as the Dallas County Health Department writes a health improvement plan for 2023-2025. The Dallas County Health Assessment Prioritization and Health Improvement Planning survey can be accessed by clicking here.
