Take the 2024 Community Health Assessment online survey

You can help make our community a healthier place!

The Dallas County Health Department is currently conducting the 2024 Community Health Assessment in partnership with the Polk County and Warren County health departments, as well as several other community organizations.

We are conducting an online survey to understand the health status of our community. The survey includes questions on several areas of health such as access to healthcare, chronic disease management, mental health, substance abuse and social determinants of health.

Results of the survey will guide health improvement activities in Dallas County and the surrounding area for the next 3-5 years.

The last Community Health Needs Assessment was conducted in 2021. At that time, the community’s top five healthy priority areas were:

  1. mental health;
  2. COVID-19;
  3. nutrition, physical activity and weight;
  4. substance abuse;
  5. and heart disease and stroke.

Both the 2021 Community Health Needs Assessment summary and the Dallas County Health Improvement Plan for 2023-2025 can be viewed on the Dallas County Health Department website.

The anonymous survey is offered online and is expected to take about 10 minutes. We value your participation!
